How do I know if I’m eligible for this plan?
You will receive an offer letter in the mail from Essential StaffCARE/Planned Administrators, Inc. (PAI) notifying you of your eligibility. The offer letter will include plan information, along with enrollment information.

How do I enroll?
The offer letter you receive in the mail will include the instructions for enrolling. You can also contact Essential Staffcare’s customer service center at 866-798-0803.
When does my coverage begin?
There is a 60-day waiting period once you become eligible for the plan. Coverage will be effective the 1st day of the following month once you complete your 60-day waiting period.
Example: You become eligible for the plan on 3/26/2024. Your 60-day waiting period would be completed on 5/25/2024, and your plan would be effective on 6/1/2024.
Do I have to use an in-network provider?
You are not required to use an in-network provider, however, it is more expensive to use an out-of-network provider. Members will receive a Preferred Provider Organization (PPO) discount when they use in-network providers. Do I have to pay for the full premium?
No. ESSG follows the Affordable Care Act’s affordability requirements. What you earn per hour will determine what your portion of the premium will be each month.
When will I receive my ID card?
ID cards will be generated and mailed as soon as your enrollment has been processed. If you need to use your insurance before you receive your ID card in the mail, please contact ESSG’s Employee Benefits team at 952-767-9519 or benefits@employersolutionsgroup.com to receive a digital copy.
