ESC Bronze Plan:
ESSG offers the ESC Bronze 5500 major medical to eligible full-time employees. The ESC Bronze 5500 major medical plan meets the requirements of a minimum value plan, and ESSG follows the affordability requirements of the Affordable Care Act.*
- $5,500 deductible for individual coverage
- First Health Network for medical services. Preferred Provider Organization (PPO)
- Premiums are collected pre-tax
- Preventive Care Screening and Immunizations are covered 100% by the insurance company
- Child(ren) are eligible through age 26. Spouses are not eligible for this plan
*If you have qualified for the ESC 5500 Bronze Plan, you may not be eligible for subsidies on the marketplace. For plans available through the marketplace, visit www.healthcare.gov.
Eligibility Requirements:
- Full-time employee averaging 30+ hours per week
- 18 years of age or older
- Must have a valid Social Security Number (SSN)
Timelines to enroll, cancel, or make changes to your ESC Bronze 5500 major medical plan:
- Within 30 days of initially becoming eligible for the plan
- During the annual open enrollment period
- Within 31 days of a qualifying life event (QLE) occurring. Documentation of QLE is required.
- Examples of QLE include marriage, divorce, birth/adoption of a child, gaining new coverage, losing previous coverage
